STARTING A PATIENT ON FINTEPLA
WITH ONWARD SUPPORT

Follow the steps below to start a patient on therapy.

Prescriber must complete certification and enrollment at www.FinteplaREMS.com prior to initiating therapy.
Then, follow these 3 steps:

Prescriber enrolls each patient at FinteplaREMS.com

Prescriber submits the Prescription Authorization and Patient Referral Form on the ONWARD Portal to start benefits verification
3
Patient obtains baseline echocardiogram; prescriber reports results to REMS via Patient Status Form at FinteplaREMS.com
Alternatively, the Patient Status Form can be submitted via the ONWARD Portal
